Guide to Obtaining Copies of Medical Records
If you wish to obtain copies of your medical records, please prepare the required documents and follow the procedures below in accordance with applicable laws and regulations. Eligibility and Required DocumentsApplicantRequired DocumentsPatientValid photo ID (e.g., Resident Registration Card, Driver's License, Passport, etc.)Authorized Representative1. Signed Authorization Letter (Power of Attorney)2. Copy of the patient's ID3. ID of the representative4. Document proving the relationship between the patient and the representative (e.g., Family Relation Certificate, Certificate of Resident Registration, etc.), if applicableMinor (Under 14 Years of Age)Valid ID of the legal guardian and supporting documents verifying guardianship (e.g., Family Relation Certificate) FeesFees are charged in accordance with the Korean Medical Service Act and vary depending on the number of pages and the type of medical records requested.

Proxy Prescription: Eligibility Requirements and Required Documents
According to the guidance issued by the Ministry of Health and Welfare, a prescription may be issued to a patient's representative (proxy) only under the following circumstances:The patient is unconscious.All of the following conditions are met:The patient is unable to visit the clinic due to significant mobility limitations.The patient has been receiving continuous treatment for the same medical condition.The patient has been taking the same medication(s) for an extended period.The following patients who are unable to visit the clinic due to their circumstances may also be eligible:Individuals incarcerated in correctional facilitiesPatients with severe mental illnessOlder adults with dementiaPlease note: Meeting the above criteria does not automatically guarantee that a proxy prescription will be issued. The prescribing physician must determine that issuing a prescription through a representative is medically appropriate and safe based on the patient's condition.Proxy RecipientEligible family member or authorized representativeRequired DocumentsPatient's ID Proxy's IDSigned Authorization LetterProof of relationship (if required)
